Some of our clients may find it difficult to step away from the support of our Social Prescribing team, and / or may be unable to achieve all of their goals during the time that one of our link workers is able to offer them. We are looking to grow a team of Peer Support volunteers who will work with a client 1:1 on a weekly / fortnightly basis for a time bound period in order to support them to achieve the goals set within their wellbeing plan. This might involve spending time with a client in their own home, or in the local community.

- Volunteer: Weekly, Fortnightly
- Availability: Monday AM, Monday PM, Tuesday AM, Tuesday PM, Wednesday AM, Wednesday PM, Thursday AM, Thursday PM, Friday AM, Friday PM
- Tasks: The support you provide will be tailored to the clients needs and may include:
- Supporting confidence and learning of travel routes to community activities (this may involve driving or using public transport)
- Visiting a home address to build confidence in leaving the house
- Supporting individual to use the internet so there are able to access support and services.
- Meeting individuals in community venues to support in building confidence and awareness of a local area.
- One off support to appointments
- Escalate any concerns or uncertainty to a member of staff
- To show understanding and strict adherence to Action Together policies and procedures i.e. information governance, equality and diversity, safeguarding, health & safety, fire safety, lone working.
These tasks are not exhaustive, and you may be required to undertake other duties in order to meet the needs of the clients and our service. - Training: You will be required to complete:
- Face-to-face Induction Session
- NHS E-learning
- Safeguarding Adults & Children Level 1 - Skill And Qualifications Required: - A calm, friendly, and non-judgemental nature
- Ability to communicate effectively and appropriately with adults (including the elderly and vulnerable) from various backgrounds and experiences
- Comfortable working and travelling independently with a client or to the home of a client
- Able to use own initiative, but a confidence to raise and escalate concerns
- A knowledge of or a willingness to learn about local services and groups available - Interests: Community, Disability, Health and wellbeing, Mental Health, Older people, Social care, Social Welfare
- Activities: Befriending, Lending a helping hand, Support Work
- DBS Check: Yes
