We are looking for a passionate and dedicated individual to join our team as part of the iThrive Referral Hub. Working alongside the iThrive Co-ordinator, you will play a pivotal role in triaging referrals and ensuring children, young people, and families receive the right support at the right time. Working alongside the iThrive Co-ordinator, you will help manage the referral hub by triaging referrals and responding to enquiries, ensuring that children, young people, and families are navigated to the most appropriate service within the Bolton Together emotional health and well-being offer and wider community and voluntary sector. You will help to maintain and regularly update the referral database to ensure accurate tracking of referrals, service access, and outcomes for children, young people, and families.
