Thanks to to Rochdale Borough Council's Social Care team, Action Together is pleased to launch the Home from Hospital Fund.

 

At this Meet the Funder session you will learn about the of the aims of the fund, how to apply and ask any questions you may have.

Aims of the session:

  • Learn more about the background to the fund and how to apply
  • Understand the aims of the Home from Hospital fund
  • Ask questions of the commissioner

Appropriate for:

  • Groups with an existing offer to communities at greater risk of hospital admission
  • Groups with experience of partnership working with the NHS and social care teams
  • Groups based in or working in the Rochdale Borough
