Meet the Funder - Home from Hospital fund - Rochdale Borough

Thanks to funding from the Better Care Fund, we are launching two funds which invite the VCFSE sector in the Rochdale Borough to respond to the challenges of hospital pressures.

 

Home from Hospital – Culturally Appropriate Support offers grants of up to £5k or £10k to meet the gap in the borough around support to settle in after hospital discharge which meets the needs of our diverse communities.

Home from Hospital – Prevention and Wellbeing offers grants of up to £10k or £25k to support people at high risk of hospital admission from needing to go to hospital in the first place.

At this Meet the Funder session, Rochdale Borough Council’s Assistant Director of Commissioning Asc and Prevention, Hayley Ashall, will explain more about the aims of each of the funds and answer questions.

Aims of the session:

  • Learn more about the background to the funds
  • Understand the aims of each of the Home from Hospital funds
  • Ask questions of the commissioner

Appropriate for:

  • Groups with an existing offer to communities at greater risk of hospital admission
  • Groups with experience of providing culturally appropriate support
  • Groups with experience of partnership working with the NHS and social care teams
