Rochdale & District Mind currently provides a Delayed Transfer of Care (DTOC) service where they support patients with Mental Health issues who are awaiting discharge from John Elliot Mental Health unit, they also take referrals for patients where there is not already a statutory service provision under Section 117 aftercare – therefore we have been supporting Section 2 discharges and informal discharges.

The DTOC Coordinator is based on the John Elliot unit working across the Hollingworth and Moorside Wards, attending the ward rounds and the MDT’s liaising with ward staff and clinicians regarding appropriate referrals to the service.

The Home From Hospital Worker, will work alongside the DTOC Coordinator supporting patients who have been discharged and require further initial support in the community.  The worker will provide a link across the various mental health support systems and practical issues – such as; homelessness, housing issues, referrals for deep cleans, applications for benefits, assisting patients who have no recourse to public funds, and immigration issues.

Contract: 12 months Fixed Term

Hours: Part Time (16 hours per week)

Salary: £23,201.08 per annum pro rata

Location: Rochdale
